This bug can happen only during a qualifying session with limited laps. After having making all laps possible, drivers are required to drive back in pit (Game message : enter pit to avoid penalty). If the driver decide to make a normal entry (Not using the escape button on track), when entering the third sectors one lap will be wrongfully added, effectively disqualifying the player. So with six laps allowed, you have one out lap, four timed laps, and one in lap. I tried with your settings and track, and everything worked fine for me. I see the checkered flag at then end of lap five.

I made a few more tests, and I managed to have a reproductible scenario as shown below : https://streamable.com/bul6iz The bug happens only between a certain point (maybe the AI pit entry lane ?) and the pit line. Though it doesn't happen if you press escape while being just in front of the pit line, it's really weird. It also doesn't seem to happen if we enter the pitlane normally then press the escape button.
Does it matter where your pit stall is located? If it's before the start/finish line compared to past the start/finish line? I think I saw years ago that if your pit is past the finish line something similar to this would happen.
I made a few tests, the pit stall position doesn't seem to change anything. I'm not 100% sure though, my pit stall in Circuit Gilles Villeneuve was very close past the start/finish line. On a side note @Coutie, the Ui has small bug for the number of laps in qualification. Not game breaking, it's just the value for an illimited number of laps.
I've did a number of tests on my computer and so far I have not been able to get this same bug. I made sure I had Flag rules to full. Tested offline and online with a 3 lap qually. It's very odd.
